New Student-Athlete Resources

All new student-athletes must have ALL of the following completed prior to participation in varsity practices and/or competition:

1. MEDICAL HISTORY & PHYSICAL FORM

(to be completed by the student-athlete)

2. INSURANCE INFORMATION FORM
(to be completed by the student-athlete) PLEASE TYPE DIRECTLY INTO THE PDF. HAND WRITTEN FORMS WILL NOT BE ACCEPTED.

3. PRESCRIPTION MEDICATION HISTORY FORM
(to be completed by student-athlete) - and pertinent documents from physician if taking medication for ADHD

4. SICKLE CELL TESTING WAIVER
(to be completed by the student-athlete)

5. HEAD INJURY REPORTING
